Basic Services

We assist individuals with disabilities to live as independently as possible in their own homes and communities. Our SIL services include assistance with daily activities such as bathing, dressing, and meal preparation, as well as help with transportation, medication, and other tasks. We enable individuals with disabilities to live as independently as possible, while still receiving the support and assistance they need to manage their daily lives. SIL can be provided in a variety of settings, including private homes, apartments, or group homes, and may be tailored to meet the individual needs and preferences of the person receiving the services.

We provide temporary housing and assistance to individuals with disabilities. This type of support is often provided to individuals who need temporary housing due to a crisis or transition, such as a hospitalization, a change in living arrangements, or the need to access medical or other services. Our short-term accommodation and assistance service include help with daily activities such as bathing, dressing, and meal preparation, as well as access to transportation, medication, and other resources. Personal care support may be provided in a variety of settings, including in the individual’s home, in a residential care facility, or in a shared accommodation. The goal of personal care support is to enable individuals with disabilities to maintain their personal hygiene and appearance, and to support their independence and autonomy in daily life. Personal care support will be provided by our trained support workers or may be self-directed, depending on the needs and preferences of the individual receiving the services.

Shared living support service may be provided in a variety of settings, including private homes, apartments, or group homes. The goal of shared living support is to enable individuals with disabilities to live as independently as possible, while still receiving the support and assistance they need to manage their daily lives. Shared living support service may include help with personal care, such as bathing, dressing, and grooming, as well as assistance with transportation, medication, and other tasks. Shared living support service will be provided by trained support workers or may be self-directed, depending on the needs and preferences of the individual receiving the services.

We support individuals with disabilities to access transportation to participate in activities and events outside of their home. This may involve providing assistance with planning and organizing transportation, as well as providing transportation directly. Travel/transportation assistance will be provided by our support workers or may be self-directed, depending on the needs and preferences of the individual receiving the services. SUPPORTED INDEPENDENT LIVING

Supported independent living is a type of NDIS support aimed at facilitating your ability to live in your own home. It includes help or supervision with everyday tasks with the goal of increasing participant’s independence and skill development.

At Hi-5 Care, our Supported Independent Living focuses on meeting the individualised daily needs of participants living in their own homes or shared housing while learning new skills that allow them to live independently. We provide a Person-Centred Active Support that is available 24-hours a day, and 7 days a week to carry out your daily tasks in a way that enables you to enjoy the freedom of independent living.

Our team of trained support professionals support people of all ages and needs to live a happy life and participate in their communities.

COMMUNITY ACCESS

Connecting and socialising with friends, families, and the community while participating in activities that support life skills development and community participation leads to a better, healthier, and happier way of life.

At Hi-5 Care, our objective is to promote inclusion and overall wellbeing of our clients by providing support to our clients to access the community and actively participate in activities that enhance their independence, quality of life, and full participation in the community.

We will support you to actively participate in social, cultural, recreational, educational, and vocational activities following your needs, preferences, and goals.

We have team of support professionals from different background and interests to support you in a variety of settings with activities of your choice and interest such as;

  • Connecting with family or friends
  • Shopping, volunteering, gardening, art and crafts
  • Attending fitness classes, sport events, library, cooking class, music and dance groups
  • Attending special events; festivals, parades, cultural events, live concerts, and more.

We can also support you with planning and organizing activities of your interest – You just tell us what your hobbies and interests are and where you would like to go

 

TRAVEL AND TRANSPORTATION ASSISTANCE

We support individuals with disabilities to access transportation to participate in activities and events outside of their home. This may involve providing assistance with planning and organizing transportation, as well as providing transportation directly. Travel/transportation assistance will be provided by our support workers or may be self-directed, depending on the needs and preferences of the individual receiving the services. The goal of travel/transportation assistance is to enable individuals with disabilities to participate in activities and events in the community, and to support their independence and autonomy in daily life.

Our support workers will provide transportation assistance for you to locations of your choice including the local neighbourhood, a medical facility, your friend’s or relative’s home, shopping mall, movies, etc. while ensuring that you enjoy the freedom of independent living.

SUPPORT COORDINATION

This level of support is a capacity-building support system designed to empower the individual through choice and control of the services they receive in all aspects of life.

At Hi-5 Care, we are committed to supporting you to maximise your NDIS plan by connecting you to the right services and support. We provide short and long-term supports that focus on strengthening participants ability to flourish and participate. We will help you outline your specific needs and goals and assist you to coordinate and manage supports dependent on these needs. GROUP AND CENTRE-BASED ACTIVITIES

Group activities are interactive and engaging exercises that involve participants working together in a group setting or rotating through different activities. These activities encourage and promote collaboration, social interaction, and skill development. Our team of support professionals can help participants with any group activities of their choice such as cooking skills, House chores and self-skills, art and craft, outdoor games, team sports, problem solving activities, build projects, aerobics, yoga, music or dance performances, outings, and more to learn new skills and build capacity to increase your independence. We also can pick up participants at their homes to participate in group activities of their choice, and then safely return participants back to their homes

HOUSEHOLD TASKS

Having a disability can present with unique challenges that may require extra help around the house. As a result, individuals with disabilities often rely on various forms of assistance to navigate their living environment to maintain a clean, organized, and functional home as well as their independence.

Hi-5 Care will provide you with support to get essential tasks done either temporarily during periods of increased need or on an ongoing basis to ensure a safe and comfortable living environment.

Our household tasks services include:

  • – Meal preparation: Planning and cooking meals for the family, including grocery shopping, meal planning, and food storage.
  • – Cleaning: sweeping, mopping, vacuuming, dusting, and wiping surfaces.
  • – Laundry: Washing, drying, folding, and ironing clothes
  • – Organizing: Sorting and organizing items in the house, such as clothes, books, toys, and kitchenware.
  • – Taking out the trash: Emptying waste bins and disposing of garbage in the appropriate manner.
  • – Gardening: Maintaining the garden by watering plants and pruning
  • – Financial management: banking, paying bills, and budgeting.
  • – Home decorating: Arranging furniture, hanging artwork, and maintaining the aesthetic appeal of the home.
